Distinguished Alumni Nomination Form
The Foundation for Daviess County Public Schools recognizes Distinguished Alumni every year.
To be considered for Distinguished Alumnus, the following criteria must be met:
- Nominee must be successful in his/her chosen field
- Nominee must have graduated more than 15 years prior to nomination from a DCPS high school
- Nominee has no criminal record
Your nomination will go directly to our Executive Director who will submit your nomination to the Foundation's Alumni Relations Committee. The Alumni Relations Committee will make recommendations to the Foundation's Board of Directors for consideration. All nominations will be submitted to the Board of Directors.
When evaluating a nominee, it is imperitive to have as much information about the nominee as possible. Feel free to composed your nomination in WORD then copy and paste into the nomination form below.
